Are you a student with a learning or physical disability?

 

Accommodations can be arranged for students who have documented disabilities. Students must self-identify and request assistance. To get the process underway, this check-list should be helpful.

  1. Gather documentation. This can include IEPs from your high school, letters including diagnosis from a medical doctor or a psychologist, or materials from a private testing agency. The cost of testing is the responsibility of the student.
  2. Make an appointment. Call 872-2440, email mmeland@oakmail.peru.edu, or stop by the CATS, room 102, to visit with Marie Meland. She is the campus ADA Compliance Officer.
  3. Complete an initial interview. This will happen during your appointment. It is a time when you will discuss what accommodations you need, what you have used in the past, and what you found helpful. It is also a time to complete the paperwork that is necessary when you apply for accommodations.

After your accommodations have been approved, letters will be sent to your faculty members, your advisor, and you. It is your responsibility to meet with the ADA Compliance Officer at least once a month to review progress and to be certain you are using the accommodations and other services available through the Center For Achievement And Transition Services appropriately. You should also keep in touch with your advisor and faculty members.
